Advancing Health Innovation for New Yorkers Too Often Left Behind
The EngageWell Network brings together nonprofit health and human service providers, mission-driven digital health innovators, and public health partners to design and test new models of care that improve health outcomes for vulnerable and marginalized communities across New York City.
The EngageWell Network operates through two integrated functions:
Community Health & Innovation Collaborative (CHIC), 501(c)3
EngageWell CHIC is the organization’s non-profit arm that advances new models of care through innovation, evaluation, and cross-sector partnerships.
Core Functions:
- Pilot programs to improve population health and reduce disparities
- Program evaluation and quality improvement
- Technical assistance and partnership development
Independent Provider Association (IPA)
EngageWell’s IPA supports community-based providers in delivering coordinated care and advancing value-based Medicaid innovation.
Core Functions:
- Value-based contracting and health payer partnerships
- Care coordination and infrastructure
- Medicaid strategy and system navigation
Together, EngageWell IPA and CHIC design, test, and scale more accessible, equitable, and effective models of care for safety-net communities.
